The terminal in which shipment entered Bob's system.

Destination: The terminal from which shipment will be
delivered.

Biller: The biller initials.

Shipper #: The shipper's bill of lading number (if provided).

Pieces: The number of pieces in the shipment.

H/M: An "X" will be marked if it is a hazardous
material shipment.
14. Description:
How the shipment is packaged (pallet, skid, etc.) and the
description of the shipment, including: description of
articles by NMFC code; arbitrary charges for delivery to an out of
the way or difficult to reach location; any discount, if applicable
and other necessary information. |

Signature: The legible signature of the person receiving the
