Your delivery receipt is a great source of shipping information.

1.  Bill Date:  The date the freight bill was issued.

2.  Pro #:  The number assigned to your shipment.

3.  Consignee:  Consignee name, address and zip code.

4.  Shipper:  Shipper name, address and zip code.

5.  Bill to:  Bill to name, address and zip code.

6.  Advanced Carrier:  Advanced connecting Line (C/L), C/L Pro # date.

7.  Beyone Carrier:  Beyond connecting Line (C/L), C/L Pro # date.

8.  Origin:  The terminal in which shipment entered Bob's system.

9.  Destination:  The terminal from which shipment will be delivered.

10.  Biller:  The biller initials.

11.  Shipper #: The shipper's bill of lading number (if provided).

12.  Pieces:  The number of pieces in the shipment.

13.  H/M:  An "X" will be marked if it is a hazardous material shipment.

14.  Description:  How the shipment is packaged (pallet, skid, etc.) and the description of the shipment, including:  description of articles by NMFC code; arbitrary charges for delivery to an out of the way or difficult to reach location; any discount, if applicable and other necessary information.

15.  Class:  Freight classification, if applicable to billing.

16.  Weight (lbs.):  The weight of each commodity.

17.  Rate:  The rate of the shipment.

18.  Charges:  The extension of rate or the calculation of charges (only shown if the shipment is collect).

19.  Delivered by:  The driver's signature.

20.  Date:  Date delivered.

21.  Time:  Time of delivery.

22. # of Pallets:  Total number of pallets delivered.

23.  SW Pallets intact:  Did shrink-wrapped deliver intact—yes or no.

24.  # of Pieces:  Total number of pieces delivered.  If on a shrink-wrapped pallet, STC (said to contain) number of pieces.

25.  Signature:  The legible signature of the person receiving the shipment and the date received.
