Date: Enter the pick-up date of the shipment.
2. To: Consignee: Enter the name, complete street address (no P.O. boxes), city, state, zip code and phone number of customer receiving the freight. 3. From: Shipper: Enter the complete street address, city, state, zip code and phone number of your company as the originator. 4. Shipper's PO #: Enter your shipper's number or your purchase order # here. 5. Beyond Carrier: Enter the name or SCAC code of the carrier that you wish to handle this shipment if going beyond the Bob's service area. 6. Special Instructions: Enter any special delivery instructions for this shipment. 7. Third Party Name/Company: Enter the name, mailing address, city, state, zip code and phone number of the third party responsible for the charges on this shipment. 8. Charges: Check the appropriate box to show whether the freight charges will be prepaid by the shipper or collected by the consignee. If no box is checked off, Bob's assumes the charges are prepaid by shipper. 9. C.O.D.: Enter the dollar amount of the C.O.D. Check the appropriate boxes on type of payment and how the C.O.D. fee is to be charged (ppd. or col.). Also enter the letters C.O.D. in the box before the Consignee name. |
10. No. Shipping Units: Enter the number of pieces being shipped for each item. 11. HM: If the commodity being shipped is hazardous material, mark an X in this block. 12. Description (List hazardous materials first.): Enter the name of the commodity and its description. This section is used in rating the shipment. Describe your shipment using NMFC. If the NMFC is unknown, then provide a complete description of commodity name, material, product use and packaging. 13. Weight: Enter the complete weight for each item being shipped. Enter the total weight on the bottom. 14. Declared Value: Enter the agreed value when the rate is dependent upon it. 15. Section 7 Liability: This block contains the "without recourse" clause. Signing this instructs the carrier to withhold the shipment until consignee pays the freight charges. 16. Shipper's Certification: Enter the company name of the shipper and signature of the person preparing the B/L. 17. Driver's Signature: This block is where the driver will sign, date, and put the total number of pieces he has received. |
